Below materials and information about the Connect To Work programme delivered across North Yorkshire to anybody aged 18+ and experiencing a health condition, disability or facing disadvantage. The programme is looking for referrals from any organisations supporting people eligible, and is also keen to promote its services at any events which may be relevant. Local Area Coordinators work with individuals and families of all ages and abilities. They help raise awareness of available resources within the local community and support people with a wide range of issues.

SOCIAL PRECRIBERS YORK– Help can be given for anyone who has a social issue that is affecting thier health and wellbeing. Direct line for the Social prescribers for the following practices, (WONE, PMG, Jorvik, My Health and Elvington) which is provided by York CVS. Pharmacies can ring on behalf of the patient while they are with them, or they can self-refer age 18+ . If they are NOT a patient of one of theses practices please still refer using this pathway and they will contact the patient to advise – Tel 01904 437911 please leave a voicemail message. Patient must give permission verbally as part of the message. Require patient name and practice and a contact telephone number to contact patient.
