Invoicing for Public Health enhanced services from April 13

Claim forms are bespoke for each pharmacy and there has been a delay in sending these out, further details from the local authorities below; 

North Yorkshire County Council;(issued 3/5/13)

The claim form will be bespoke for your pharmacy and will need to be returned to the following e mail address:

Publichealth.claims@northyorks.gov.uk

You will be able to use the form to claim for Smoking Cessation, EHC, Supervised Consumption and Needle Exchange.

York City Council (issued 1/5/13)

Due to the delay in getting the forms to you, we have decided to extend the deadline for claims for this month only to Friday 10th May (it was previously 8th May due to the bank holiday). We will then check and process the claims, with payment still due for the end of the month. Please note the claim forms have changed slightly since the draft versions were issued and a number of technical problems within the spreadsheet have been resolved.  Once you receive the final version of the forms tomorrow, please delete any previous drafts. In order for us to ensure that the new payment process is as efficient as possible, where possible we are requesting that claims for April are submitted using the electronic claim forms.  If you have any queries or need any help in completing the forms, please do not hesitate to contact us.  Contact details for the claims process will be issued with the forms tomorrow.
