York Public Health contract update

Dear Pharmacist,

Hope you enjoyed the bank holiday weekend.  Please see attached an updated draft of the Public Health Contract, which has been amended to populate gaps and to finalise clauses and schedules which were still under review.   For reference, attached are a clean and track changed version showing all the changes we have made since the previous version was issued in March.  If you have any questions please let us know, otherwise we will proceed to prepare the final contract for your pharmacy for signature.  Pharmacies which have multiple branches in York will be able to sign a single contract for all branches, with appendices showing the services for each branch. Please note that the contract at this stage has two things missing:

  1. The specifications at Schedule 1: these will be added to the contract for your pharmacy (or company if multiple branches)  to correspond to the services you have signed up to deliver.
  2. The claim form at Schedule 2B: this will also be added to the individual version for each pharmacy (or company as above).
